Proof of Vaccination or Negative Test

Aloha Pearl City and Waipahu Families,
This letter is to inform you that all visitors and contractors must show proof of COVI D-19
vaccination or a negative COVID-19 test result prior to conducting business on a Hawaii
Department of Education school campus effective Monday, September 13, 2021, in accordance with Governor lge's Executive Order No. 21-07 (Access to State Property).
Please check in at the school's front office and be ready to show a photo ID with either of the following:
• Proof offull COVID-19 vaccination, or
• Proof of negative COVID-19 test taken within the last 72 hours.
All visitors and contractors will be denied access to campus and DOE facilities until the above mentioned documentation is provided.
