Aloha e Waipahu Intermediate Parents and Guardians,
1). Please go to the following link and select WAIPAHU INTERMEDIATE SCHOOL in the drop down menu to register your child for the COVID vaccination clinic. You may also register any of your other children who are 12 and older and attend a Waipahu Complex School:
2). Open and print the following link. THESE FORMS MUST BE COMPLETED AND BROUGHT TO YOUR CHILD'S APPOINTMENT:
3). See the following vaccination site map for parking and other direction
- Copies of these forms can be picked up from our front office if you do not have a printer.
- If your child will be in school that day, make sure you send your child to school with these completed forms.
